Mark Masselli and Margaret Flinter speak with Dr. Lucian Leape, one of the country's leading figures in patient safety whose body of work includes the landmark Institute of Medicine report "To Err is Human." Dr. Leape currently chairs the Lucian Leape Institute at the National Patient Safety Foundation and teaches at the Harvard School of Public Health.
